COPIES OF REQUIRED DOCUMENTATION FOR ENROLLMENT

  • Student Legal Name and Age

    Birth Certificate

  • Immunization and Physical Exam

    Proof of immunizations on Form 680, obtained at the Department of Health in Lake County or through your private physician.

    Proof of physical examination by a U.S. doctor within a year of enrollment (first day of entry at school)

  • Special Program Information (if applicable)

    Current IEP

    Current EP

    Current 504 plan

  • Residence

    Any one of these documents:

    Valid Florida Driver’s License/ID with a current physical address

    Florida Voter’s Registration Card with the current physical address and a photo ID of the parent or guardian registering the student

  • Guardianship

    Birth Certificate

    Court Custody Documentation
